How to Submit a Request Using
the Library Materials Request Form
Use the Library Materials Request Form to request any item that is not available
on Baker & Taylor Title Source II.
Before using this form, please check to be sure the item you are
requesting is not available on Baker & Taylor Title Source II. If the item you
wish to order is from a sale flyer or catalog, then use this form to submit your request
and send the flyer or catalog to Christopher Rico via the campus mail.
Requests are submitted electronically to the Acquisitions Department where they
are checked for duplication, charged to the appropriate academic department and
fund and submitted to the appropriate publisher or book jobber.
LogOn
At the Library Materials Request Form, you will use your e-mail login and password
to access the form. If a student will be doing your ordering for you, you will
need to send him/her to the library for Christopher to put them on the authorized list to use the form as it will not accept student id or password.
Enter Data
Once on the form, fill in all the information that you know. The form will
require that you fill in the author, title and either the publisher or the ISBN
before submitting it. Some suggestions for supplying this information are
listed below:
- Author -- if the author is not available, then try the editor, organization,
director, composer or performer
- Publisher -- if the publisher is unknown, then try the organization or
production company
- ISBN -- for books from catalogs, videos, CDs etc., if the ISBN is not
available

Submit Request
When you have filled in all the necessary information, then click on the SUBMIT
button at the bottom of the page. Your request will be sent directly to
Acquisitions.
